Need replacement Double Wheel Assembly

Question

I have a Double Wheel Assembly with a seized wheel. I need recommendations for a replacement. Measurements: 5 3/4" installation hole center to the opposing installation hole center; 1 1/2" base of the unit to the top of the unit; 3/4" wide.
