Need replacement for Top Sash

I recently ordered S380-30 : 30" Series 380 Channel Balance. While replacing the balance however I noticed that the top sash is also broken. I looked through the website but couldn't find any top sash that has two holes. Attached is the photo

It seems photos didn't attach properly. trying again
