It looks like you have either our S351-29, with a 15-002 bottom attachment, or S390-29, with a 15-001 bottom attachment. Take a look at the image below to determine which balance you'll need. You'll also need a spring strength option of 2830.

As for your top sash guide, do you have an undamaged version you could show us? If not, could you try and stick the broken piece together and show us?
Take a look at our 18-199. Look over our dimensions and compare them to your current top sash guide to see if this matches up for what you need. If the dimensions differ in any way, please let us know exactly how. We have other options if the 18-199 doesn't match up.
