Thank you for reaching out to us. This looks to match our S770-23. Your stamp doesn't match any of our options so you'll have to weigh the window sash on a scale to determine the proper option. There's a chart under the product description on the S770-23 store page that you can use as a reference. Please note that the Series 770 installs with a screw so you'll want to make sure your balance does too. 

As for the pivot shoe and pivot bar, take a look at the 15-135 and 26-377. Review the specifications for these parts and see if they match what you're searching for.
