Need to find right part

A quick learner from Bothell, WA says:
The code on my channel is "26-4 LE5 CMC 75". The length of the channel is 27 inches.

Can you please share the right part number that I should be ordering.

Attached are the photos of both the ends as well as cord and hook, that's the part broken and need a replacement because of that.

Thank you for the additional pictures! This is a huge help. 

With this combination of attachments, you could have either an S380-27 or S385-27 channel balance. You’ll need to measure as shown below. The Series 380 has a 1" bottom attachment, and the Series 385 has a 1-3/32" attachment. You'll also need a spring strength option of 2640.
