Need to identify correct window channel balance model- for NON-Tilt window

Please help me to identify the correct model window channel balance. I do not seemed to be able to look up and select the correct ones (model like before). I have looked through models 350, 351 and several others, but I am not able to determine the header and footer. My 4 photos should show

1. overview

2. the stamp identifier

3. the header close up

4. the footer close up

Added note:
Here is what I had determined, but was not for sure. Please help.

1. Stamp on the body reads "26 5 GW CMC 11". This means it is a 27" metal body. The channel's width measures 9/16" but is considered 1/2" in the trade.
2. Per the balance parts header and footer both have wings. Maybe it is parts 15-004 and 19-005.
3. The weight balance is 50.

Thanks for contacting us. I'm afraid your photos didn't come through, so I do not have a reference for your post. I can tell you that if you have a 19-005 top and 15-004 bottom, then you want our S385-27 with a 2650 option. However, I cannot confirm this without photos.

Thanks! In terms of style, your nylon attachments do look similar to our 19-005 and 15-004. I would just confirm that your dimensions match up.
