Need to order Channel Balance Parts

Need help identifying the correct channel balance and top end pieces to purchase for a PGT single hung window. the balance is 30" long and 5 1/6 wide. I have a couple of pictures attached.

For your PGT window, the top sash guide that should replace yours is our 18-015. As for the balance, see if our S395-30 looks about right. We will need for you to get back to us with the stamped number on your balance in order to determine the weight range option.
