It appears that your window uses the Series 770 channel balance. Please note that this balance installs at the top with a screw. It looks like your balance installs with an S-clip at the top like the 19-029 or 19-030. If so, we will need to assemble a rivet at the top of the balance for you via a custom order.
To move forward, we will need to know the length of the metal channel along with the number/letter stamp on the metal. How many balances would you be in need of? For custom orders there is a minimum of (2).
For the pivot shoe, please start by reviewing the style and dimensions of the 15-135. Compare to your original to confirm a good fit! Let us know if you have any additional questions.

I have sent over an order link for (4) balances with pricing for your review. The balance will be 16" long with a 16E stamp that can support a 19 to 22 lb window sash. We will install a rivet at the top to accept a S-clip for installation.
Please note that the pivot shoe and s-clips will need to be added to your cart additionally from the product page if needed before checking out.
