The stamp printed on the balance doesn't help us identify it, unfortunately. Can you remove an undamaged balance assembly and show us some photos? The related video on this page will go over the details we're looking for.

Along with the balancers, how do I order the plastic parts that are on the sides of the sash? They should probably be replaced as well.
It sounds like you're referring to the top sash guides. We'd be happy to help with that but the balance's identity doesn't tell us which one you may have. For us to help, we will need to see an undamaged top sash guide removed from the window. Seeing the side profile and a shot of the front of the guide would be a great help as well.
