Thanks for reaching out! Our 15-228 Pivot Shoe should work to replace the one you have. Yours appears to be an older discontinued version of this shoe. Carefully review and compare the specifications below to confirm these are a match.

Dimensions for 15-228

The compatible coil springs are the 1/2" wide Series 160 or 3/8" wide Series 165 Coil Balances. You'll need to measure the width of your coil to confirm the correct replacement. 

The S160 is not available in an 8lb coil, so you'll need either the S160-07 or the S160-09 to replace it. This will depend on the sash weight. The S165 can be replaced with the S165-8.

The plastic part is not included with the coil spring but we may have one we can recommend.
