This looks the ROTO 39-091 and 39-092. Unfortunately, ROTO has stopped manufacturing a lot of the hardware needed for these operators, including the crank handles, and soon even the operators themselves will be out of stock.

As for your hinges, take a look at our 36-048 hinge set and see how the dimensions compare. Thank you!
