I'm not sure if any of this will be helpful for you because we don't know the manufacturer of our sliding glass door or how old it is. It could be original to the house which was built in 1954. The handle is separate from the lock and the lock broke off today. You essentially lift the lever to unlock it and pull on the handle to open the door. I can't seem to find this style anywhere and I'm not sure what to replace it with. I wish I could be more descriptive, but I can't find any special markings on the lock or the sliding glass door for that matter. Any info you can provide would be helpful.

Hi Kayie. This is an interesting setup, as it looks a lot more like the locking handles used on some casement windows. With that in mind, check out the 38-001, 38-005, or 38-036. Review their dimensions, compare with your old locking handle, and see if you think one of these might work as a replacement.
