How do you replace just the roller , or does the entire bottom rail of door need replaced

Yes. The video is for regular roller replacement

The door I'm trying to figure out in picture does not have a roller style cartridge. They are built into the bottom rail of door.

In your case, you'll need to lay the patio door on top of a set of saw horses or a table that is stable enough to hold the weight of the door. Next, you will need to remove the bottom rail of the door. To do this, remove the screw at each end of the door that holds the frame together. After the screws at each end are removed, use a block of wood with either a mallet or hammer to gently tap off the bottom of the door. With the bottom rail removed, you should be able to access the roller. Once you are able to remove the roller, take a look at the 81-133. I can't be sure, but I suspect that this may be the part you are looking for.

Thank you, Yes that is exactly what I was looking for. Wanted to make sure before I removed the bottom rail of the door.
