Operator is marked truth 30905 and 40352. Two screws driven in at angle. Operator is 4" w by 5/8 thick.

Jam bracket is marked Roto no numbers. Jam bracket 9 9/16 x 11/16. Sash bracket 7 13/16 x 7/16. Need both R and L versions.

Thanks for posting. There is a good chance we can supply you with a replacement casement operator but I'll need to see a photo of the entire operator to make the correct recommendation. The body of the operator looks like one that many different operators are made with, so I'll need to see the operator arm to confirm the right replacement.

The jamb bracket you are referring to is called a casement window hinge. We likely do not carry an exact replacement for your existing hinge but we do have a few that should work fine as a suitable replacement.

Start with taking a look at our 36-048 . Carefully review and compare the measurements to determine if this could possibly be the replacement you need.

These can be used for both the left and right side of the window. You'll need to be sure to change both the top and bottom hinges to ensure your window continues to function smoothly.

Also, keep in mind, you may need to drill new installations holes. With that said, you'll need to exercise caution when drilling to holes so that you do not damage the frame and/or crack the glass pane.
