We'd be happy to help but we would need to see the attachment on the opposite end of the balance as well to identify a possible replacement. 

As for your top sash guide, you can start by carefully comparing the dimensions of our 18-015 to yours and let us know if there are any discrepancies and what yours measures. Thank you!

18-015 does not look like ours, plus the we measure one inch. Also we have 5 windows and the balance is different...the number is 2940...picture included.

Your balance looks like our S395-30. Take a look and see how they compare. As for your top sash guide, I'm going to have our senior expert take a look and see if they can offer any other recommendations. Thank you.

Do you know what PGT window series you have? The 18-015 that Casey recommended is used for their 4000 Series window. There could be minor changes from your old top sash guide to their new style. You might have to notch one side to fit your sash.

There may be a sticker on the window or inside of the frame with this information.
