Can you help me identify the correct part to purchase from you. I believe these would be a S395-30 in 2910 weight rating

Yes, your PGT window does use Series 395-30" side load balance, but the spring weight on your channel is (29 2), so you need (2920) spring weight for a sash weighing between 12-16 lbs.
