Your pivot shoe is an older, discontinued model. The good news is that we should still be able to help. Our 15-212 has a very similar design and I believe it can replace yours. However please note that its cam hole size is 5/16" x 3/16", which is fairly large for this style. Check your own cam size to confirm whether this will be a dealbreaker or not.
