I can't be positive based on your photo, but the Pan Am 18-042 top sash guide pair is a similar design. Take a look and compare the dimensions below to confirm if it's a match.

As for the balances, we can supply 18-1/2" balances, but it would need to be a custom order with a lead time for us to order the parts. To get started, we'll need to see the top fitting attached to the opposite end of the metal channel, as well as have you write out the exact depth of the bottom fitting as shown in the image below. We'll also need to know the number of balances you're looking for, with the minimum being two. Thank you!

If your balances are exactly 18-1/2" long, using a different length would affect the travel length of the pulley system, which could result in the window not being supported properly or the balance breaking prematurely. 

As I mentioned, we'll still need to see the top fitting attached to the opposite end of the balance channel, as well as have you write out the exact depth of the bottom fitting compared to the diagram we sent you. From there, we'll be happy to get a quote together for you.
