Hello there. This is most likely the 39-031 and 39-032 Dual-Arm Operators, depending on the arm lengths. See below. For a handle and cover, see the 39-114L and 39-107.

These operators aren't really distinguished that way. The 39-031 has the arms attached to the left of the spline where the handle attaches, whereas the 39-032 has the arms attached on the right side of the spline. See below. You'll need to look at your own operator to see what matches best.
