Thanks for your post! Your PGT coil assembly reminds me of the Series 210 Roller-Tilt balance. Since your housings are stamped with an 8 and a 12, you'll need the S210-20 (a pair can support a sash weighing 37 to 40.99lbs).

To replace the mounting bracket, you'll need the 16-030 Clear Bracket.
