Door Closer Part ID

Hello,
There is a door, lower level side entrance. The home is 20 years old. As you can see from the pics, water is getting in between the storm door and the wood door due to the gap from the sweep and the 1/2" variance drop from one side to the other. It's a typical 36" door. What do you recommend for a replacement to seal both of these areas?
