Thanks for posting. This coil spring looks like our S150-4 4 lb Coil Spring. The pivot shoe is a bit trickier, however; as this type of pivot shoe is really meant for windows that use spiral balances, not coil balances like yours. Likely this was some sort of custom job performed by whoever originally installed your windows. 

We recommend replacing this pivot shoe with one meant for coil balances; but to do this, we'll need the exact width and thickness of your old shoe. We also want to see the pivot bar, or the die cast pin that connects the bottom corners of the window sash to these pivot shoes. This information will help us identify the right replacement for you. Thanks.

We believe you can use the 15-219 Pivot Shoe. Unfortunately your pivot bar is not available. Try to bend it back into place. If you can't, please remove the bar and give us precise dimensions, such as the size of the pin as well as the distance between the pin that fits into the shoe and the center of each mounting hole.
