I don't believe you'll find this black piece anywhere. Your hinge isn't made anymore and these sliders weren't ever sold separately, anyway.

I recommend the newer 36-048. It's a little different compared to your original hinge but should still work as long as you replace the top and bottom at the same time. The video below will clarify how this can be done.
