It looks like you need a new pivot shoe. The bad news is that your old shoe isn't made anymore, but you can use our 15-192 with some alterations. Namely, you'll need to shave off the alignment lugs on the front face of the shoe so that it will fit in the track. You may also need to reuse your metal cam, but you could also be find with the one it comes with. I'd try it out first and see how it goes, and if the pivot bars don't fit in the new cam then you can reuse the original.
The coil springs and cover look to be in good condition, so they can be reused with the new shoe.
