It's tough to make out what you have while it's still installed, but I'm reminded of our 23-079 closet door guide. This may be what you want, but carefully compare it to the original to be sure.

You'll also note that this doesn't come with a metal bracket. That bracket is no  longer something we can supply, so you will be forced to reuse your original.
