Thank you for reaching out to us. It looks like you have a tilt window spiral balance. Depending on the diameter of the outer tube, see the 3/8" Series 900 or 5/8" Series 600.
The pivot bar housing reminds me of the 26-399. This is for the upper sash. The compatible pivot bar is the 26-400 but we recommend you compare the specifications with your old one to confirm a good fit.
We're happy to help with the pivot shoe but we'll need more information. Can you remove it from the window and show us a few photos? We will also need to know the written-out thickness and width of the pivot shoe. The video below will go over the steps to remove yours.
You pivot bar and the housing appear to be rusted so you'll need to replace both. If you'd like us to ID your pivot shoe, we'll need to see an undamaged version removed from the window along with written out thickness and width dimensions.
