Our 18-110 top sash guide resembles the old one. Take a look at the specifications and see if they align with yours. 

As for the channel balance, does the bottom shoe measure 1-1/8" deep? If so, I'd recommend the S390-20. There should also be stamped numbers on the metal channel. That will tell you what stamp option you'll need to select for the replacements.

That's a weird stamp. It doesn't match any of our options so you'll have to weigh the window sash on a scale to determine the proper stamp. There's a chart under the product description on the S390-20's store page that you can use as a reference for which stamp option to select.
