Thanks for your post! I'll need to see a photo of the shoe in the window to be able to identify it. Unfortunately, the pivot bars do not given enough information to identify the shoe.
The 26-407 Corner Pivot Bar appears to be a match for the pivot bars you have. Carefully look over them to determine if these are the replacements you need.
The window latches you have remind me of the 26-155 Internal Window Latches. They use the 26-154 Sash Release Lever.
The latches are sold in a left and right pair, but the levers are sold individually.
Thanks for the additional photos. This shoe appears to be the 15-105 Pivot Shoe. The pivot bar typically recommended for it has the same dimensions for the post that protrudes from the 26-407, so I do believe it should work for the repair.
Be sure to remove the shoe and compare its dimensions to confirm that it is the right replacement.
If you also need to replace the hook anchored inside the shoe, you'll need to replace the entire balance assembly. In that case, I'll need to know the length of its channel, the stamp on the metal, and how the balance installs at the top of the window track.
