Take a look and compare the specifications of the 82-030 handle set with 3-15/16" installation holes to your assembly, which would work with the 82-218 mortise lock shown in your photos. 

Please note that this kind of damage to the handle set usually occurs when the rollers are in bad condition and need to be replaced. It would be a good idea to check the rollers to prevent the new handle from breaking as well. Thanks for posting!
