the brass nut holding the spiral shaft in is the most critical. Also, not sure about the best tool to tighten in the brass nut.

I am afraid when that worm gear pops out with the handle, it means the entire operator has broken down and needs to be replaced. Can you show us the entire operator, including the link arm(s)? We'll do our best to see if we have any options for you from there.
