The base has two middle drawers with guides on the back. The upper drawer nylon? guide piece has broken tabs on the bottom. The best I can tell, It is stamped Ronthor Reiss Corp. part number R-40-?. The vertical height is 1 7/8". The slot dimensions are 1 1/16" wide X 5/16" high. Is this enough to identify the part and to recommend a replacement?

Take a look at SWISCO product code 32-031, drawer guide. This guide has been known to replace the guides for Pennsylvania House drawer slides, however, I am not certain they only used this shape so compare and let us know if/how they differ.
