This pivot bar (2 1/4") is bent and has also come off of where is was attached on the sash. I'm wondering which one of your pivot bars might be the best match as a replacement.
I'm also wondering what the best method for reattaching it to the sash might be as the screws have come out. Perhpas a good quality glue?

Your pivot bar looks like a good match for our 26-466 but I recommend you compare our dimensions to yours to confirm this matches up to your old one.
As for re-installing it to the sash, I'm going to have one of our senior experts who has more hands-on experience take a look and see if they could offer some insight. We'll reach out as soon as more information is available.

For vinyl sashes, there should be a metal channel inside for the screw to attach and hold in place. Yours doesn't seem to have it. We suggest you try to use a plastic wall anchors to secure the screws.
