Pivot Shoe replacement

A home owner from Massachusetts says:

I need to replace several pivot shoes in my Twenty year old Harvey vinyl double hung windows. The shoes measure:
Width 1 1/4 inches
Height 1 1/4 inches
Depth 5/8 inches

A metal hook needs to attach to it as shown in the picture.

What part do you recommend ?
