Pivot bar cover

I'm trying to find a cover for a pivot bar that broke. I'm unable to find a manufacture name. Only a tag that I think was the contractor's order information. See attached photos.

One more question. Does it simply slide out the end?

Great, thank you! We appreciate your business. As for replacement, your guess is correct. Once the sash is out, you should be able to just pull it out from the side.
