Only # is LIC 5,069,001 and a 2 in a recessed circle. Total length is 2 11/16" with 3/4" between holes.

I am having the exact same problem. What was the result from the person in Warsaw? My windows were installed during the same time frame. The pivot bars seem to be breaking very easily, now, and are in need of replacement. The picture is identical to the pivot bars I have.
