Hello there, Michelle. We in fact do offer a pivot bar with a one horn tip, our 26-454. Its mounting screw hole measurements are 1" and 2". The one horn tip should fit right into your pivot shoe. Please review the part further for additional details.

-What is the downfall of not having the double horn tip vs the single horn tip?
-Also do you off a screw that matches the one in the photo?

I don't think there is any downside to using a pivot bar with just one horn. Once the pivot bar is in the cam, they both function pretty much the same. The double horn might hold it in there a little better but I've personally never noticed a significant difference. 

You can use our 33-074 self-tapping screw to install the 26-454 pivot bar.
