We have a couple pivot shoes in this style, but I don't know what your dimensions are so I cannot identify the best match. Start by looking at our 15-178 and 15-188. One of these may be what you want.

From your photos and dimensions, I believe our 15-188 pivot shoe would be a good fit. Check it out and let us know what you think.
