Plastic slide for dresser drawers

I do not see any numbers on the broken pieces or the pieces that are intact. I see a similar part in your web site but there are 2 pieces and this has only one. They are screwed into the drawer slider that is 15-1/2" long and 1" wide. They are all installed a 1/2" in from the beginning edge of the slider. Four of these are completely broken. Hope you can help! Thank you!
