Please help ID top sash guide

Hello,

I need help to id this top sash guide. It does not appear to have a ramp like those in the photos although there is some damage at the bottom. The top of the guide is 1" x 11/16". It is 1/2" wide and 7/16 deep. It measures 5/8" from under the guide and the single hole is just short of 1/8'measured from under the top guide.

I have identified the channel balance that goes with this at S351 if that helps.
