Please help me identify this Channel Balance

If all the balances in question were installed around the same time, then it's not too surprising they're all breaking at once--the average lifespan of a channel balance is perhaps 10-15 years. It looks like you have either our S380-27 or S385-27. The only difference between these two balances is the depth of the bottom nylon shoe, or the 'heel-to-toe' measurement, so compare both with your old hardware to see what matches best. You'll need a stamp option of 2640.
