Good morning! Take a look at our Series 377 channel balance and let me know if this is a match. Just make sure that the bottom shoe is the same as yours, since I don't see that in your pictures.

Eureka...(already confirmed the bottom matches the 15-005 shoe). Buying the parts now. I'll post again with results.
