Plygem door handle replacement

Question

Trying to find a Plygem door handle assembly to replace my broken handle.
The two screws that keep the handles together broke off in one of the handles.
The handle holes seem to be 4" between centers
Applied to order number is in one of the pictures, but the last digit is not discernible.
