Push arm falls out of slider

Vantage I Casement window 287-N-022
When opened more than a few cranks, the push arm falls out of the little plastic slider, and the window will not open or close until the arm is manually re-inserted into the slider.
The installed picture is looking up at the bottom of the partially opened window, with the arm inserted correctly into the slider
