With this combination of attachments, you could have either an S380-30 or S385-30 channel balance. You’ll need to measure as shown below. The Series 380 has a 1" bottom attachment, and the Series 385 has a 1-3/32" attachment. 

Your stamp indicates that you'll also need a 2965 spring strength options for your replacement balances to support a sash between 35 and 39 lbs. To confirm, you can weigh your sash and follow the chart on the product page that explains how to choose your option based on the sash weight.

15-004 1 3/2" is what I have. The channel is 5/8" deep ? 9/16" wide ? 30" long
I'm guessing I need s385. If I'm correct I will be placing an order for 2 next week. When I place the order I ask for S385??

Yes, you'll need the S385-30 channel balance with a 2965 strength option as long as your window sash weighs exactly 38 pounds. Thanks for posting!

I have no way of measuring the window with an accurate scale. But the channel balances that are broken came with the windows from the manufacturer. They are only 6 yrs. old.

I'll have to take my chances and order the S385-30 with 2965 strength code. I'll be placing an order next week for 2 to start with. I need a total of 10 but I'll see how this works out first. Your help is greatly appreciated.
