Replace window weather strip on outside of part of window

Hi, I have several windows where the outside part of the window weather strip is tearing apart. I believe the windows are Simonton, but can't find anything on the window that lists the model. The 30" windows are about 14 years old. Looking for part to order. I included the measurements on the photo.
