Replacement Caldwell CMC 26-4 balances

Hi, I'm looking for replacement balances for a pair of windows at my house. They're stamped CMC 26-4 and are 27" in length by 5/8" (width perhaps?) and 1 1/16" depth (I took these measurements a while back, sorry). When I replaced similar window balances at my house 5 years ago the replacements came with 15P142 Winged guides on the top and 15P102 winged guides on the bottom. Hopefully you can help, and thanks!

We should be able to assist, but to further assist, are you able to provide close up images of the top and bottom nylon attachments? From there we can determine the series your window uses. Thank you!
