Replacement WIndow Balances(?)

A quick learner from Seattle, WA says:
I broke one of the balances on my window several years ago trying to remove the window sash to clean it. I removed both the working and broken balances in attempt to find a replacement, and at some point misplaced both of them. I attached a picture of the label on the window, the inside of the window frame and the side of the window itself. Can you help me locate the replacement balances I should purchase? Thanks for your help!

Unfortunately, without having the original balances on hand to use as a reference, we have no way of knowing what was installed originally. Do you have an identical window in the house that still has its balances? If so, we can use that. Otherwise, I don't believe we will be able to help.
