Replacement crank help

A home owner from Campbell, CA says:
Photos showed broken crank from lefthand Milgard casement window. Crank shaft snapped. Installed photo is from identical mirror image right hand window. Window was installed in 2006, consistent with the date stamp on the part. If pictures aren't clear, there are eight mounting screws on the base and three screws secure the arm to the window. Base plate is five inches wide.
I need a part number and would appreciate a quote on a replacement part if you carry it.
