Replacement door handles

Question

Looking for replacement door handles for two matching sliding glass doors. The only identifying information is on the glass itself: PPG Herculite K with the following numbers:
CGSB12GP1
16CFR 1201C11
ANSI z-97.1-1984
SGCC-250 60211495

The spacing between the screws in the handle is 4"
The actual lock assembly is 3" tall.
