Replacement hinges

We need to replace cabinet door hinges. The portion that attaches to the cabinet door is 2 & 3/4 inches wide. The screw hole slots on that piece are approximately 2 inches on center. The indentation portion that goes into the door measures about 7/8 inch wide by 1 & 1/4 inches long. The portion that attaches to the cabinet frame is just under 1 & 1/4 inches in length. The numbers on the hinge appear to be J3030.67. They also have plastic inserts that seem to have broken off, not allowing the doors to close property. Do you carry this hinge?
